Dear Parent/Guardian of Norwood 6th grader,

Ohio’s immunization requirements changed for the 2010-11 school year, progressing into the 2013-14 school year. Before entering the 7th grade, students must have received a booster dose of the Tdap or Td (adult vaccine for Tetanus, Diphtheria, Pertussis).  Your current 6th grade student will need to show documentation of having received this booster dose by the time he/she returns to school in the fall.

Any student who completed the original DTP/DTaP/DT series and received a dose of Td or Tdap for an injury or other reason within 5 years of entering 7th grade , (on or after August 21, 2008), will meet this requirement and not need to be re-immunized for entry to 7th grade.

If your child has not received the vaccine, please contact your child’s doctor/clinic to schedule an appointment.  Norwood Health Department will be available throughout the summer to administer the vaccine to your incoming 7th grade student. The NHD (458-4600) holds immunization clinic on the second and fourth Monday of each month from 2-4pm. Please bring your child’s immunization record with you.

 Section 504 of the ADA Amendments Act of 2008 ADAAA:

Section 504 of the Rehabilitation Act of 1973 requires a public school district to identify each child who has a disability that substantially limits a major life function such as learning, eating, sleeping, etc. If you believe your child’s health condition substantially limits a major life function, he/she may qualify for an evaluation to determine Section 504 eligibility.  Please feel free to contact the Norwood City Schools’ Special Education Coordinator, Joe Miller, at 924-2502 or Norwood City Schools’ district nurse, Kathy Strasser at 924-2882.
